What you need to know
The Admissions Department is located on the first floor of the hospital. The office is open 6 a.m. to 8 p.m. Monday through Friday and 7 a.m. to 3:30 p.m. Saturday and Sunday. After hours admissions are handled by the Emergency Department.
To help ensure your admission is as convenient as possible please have the following documentation with you upon arrival to Henry Medical Center:
Current address and phone number
Insurance card, Medicare card, Medicaid card, or Workers Compensation billing information
Written orders from the physician (must include: test ordered, diagnosis, physician’s signature)
Pre-certification and/or referral number if required by your insurance company
Should you need assistance to communicate (hearing, vision, or foreign languages), please let us know.

Advance Directives
The best way for you to be in control of your medical treatment is to record your preferences in advance. You can make legally valid decisions about future medical treatment through an Advance Directive. In completing an Advance Directive, you can do two things: legally appoint someone as your Personal Agent for Health Care to make healthcare decision for you when you cannot speak for yourself and/or formally state your wishes for the medical treatments you do or do not want to receive.
You may request an Advance Directive document during admission, from your nurse, or from Pastoral Care at 678-604-1054 or Quality Management at 678-604-1056.
If you have made your wishes known, it is our policy to honor a patient’s Advance Directive or any other health care directive that meets the requirements of Georgia law.
